ABSTRACT

ABSTRACT The aim of this study was to determine Vickers microhardness (HV) in bulk fill resins at different depths. Test specimens were prepared with different bulk fill resins: Filtek Bulk-Fill (3M ESPE) [FBF], Surefill SDR flow (Dentsply) [SDR], Fill-UP (COLTENE) [FU] and Surefill (Dentsply) [SF]. Semi-cylindrical test specimens were prepared in a mold 6 mm in diameter and 4 mm thick (n=5). A 1000 mW/cm2 light curing unit was applied (Coltolux LED - Coltene) for 20 seconds. HV was determined with three indentations (Vickers Future Tech FM300, 300 g, 8 s) at four depths: 1, 2, 3 and 4 mm from the top surface to the interior. Data were recorded immediately (t0) and 24 hours later (t24). Results were analyzed with two-way ANOVA (p<0.05), and multiple comparisons were performed using Tukey's test. Mean and SD of HV at t0 for each mm were: [FBF] t0: 49.23(4.65) / 48.32(3.36) / 44.38(2.06) / 40.59(2.58); [FBF] t24: 61.37(3.47) / 62.63(3.03) / 57.27(5.22) / 56.37(5.88); [SDR] t0: 27.81(3.13) / 28.07(2.4) / 27.24(2.94) / 25.71(3.0); [SDR] t24: 35.11(2.16) / 35.17(1.96) / 35.53(1.81) / 33.18(2.08); [FU] t0: 41.43(1.41) / 39.87(0.88) / 38.11(1.81) / 39.09(1.92); [FU] t24: 49.27(1.54) / 48.77(1.77) / 48.65(1.88) / 46.76(4.93); [SF] t0: 71.35(7.09) / 67.39(9.76) / 68.95(6.21) / 64.1(8.35); [SF] t24: 76.06(6.61) / 75.31(9.37) / 75.2(11.57) / 69.81(12.14). ANOVA showed significant effect of material, depth and recording time (p<0.05), and Tukey's test showed that recording sites (depths) differed significantly, giving four homogeneous groups. Under the conditions of this study, it can be concluded that microhardness of bulk-fill resins can be affected by depth and post-curing time.


RESUMEN El objetivo del presente estudio fue determinar la microdureza Vickers (HV) en resinas bulk-fill a diferentes profundidades. Se confeccionaron probetas semicilíndricas de 6 mm de diámetro y 4 mm de profundidad con diferentes composites de aplicación en bloque (Bulk-fill): Filtek Bulk-Fill (3M ESPE) [FBF], Surefill SDR flow (Dentsply) [SDR], Fill-UP (COLTENE) [FU] y Surefill (Dentsply) [SF]. Se polimerizaron con Coltolux LED (Coltene) con 1000 mW/cm2 durante 20s. La HV se determinó realizando 3 indentaciones con 300 g durante 8 s a 1,2 3 y 4 mm desde la superficie de la probeta hacia el interior inmediatamente después de curada y a las 24 h. Se utilizó un microdurómetro Vickers Future Tech FM300. Los resultados se analizaron estadísticamente mediante ANOVA de dos vías y Prueba de Tukey. La media y DS de HV fueron: [FBF] t0: 49,23(4,65) / 48,32(3,36) / 44,38(2,06) / 40,59(2,58); [FBF] t24: 61,37(3,47) / 62,63(3,03) / 57,27(5,22) / 56,37(5,88); [SDR] t0: 27,81(3,13) / 28,07(2,4) / 27,24(2,94) / 25,71(3,0); [SDR] t24: 35,11(2,16) / 35,17(1,96) / 35,53(1,81) / 33,18(2,08); [FU] t0: 41,43(1,41) / 39,87(0,88) / 38,11(1,81) / 39,09(1,92); [FU] t24: 49,27(1,54) / 48,77(1,77) / 48,65(1,88) / 46,76(4,93); [SF] t0: 71,35(7,09) / 67,39(9,76) / 68,95(6,21) / 64,1(8,35); [SF] t24: 76,06(6,61) / 75,31(9,37) / 75,2(11,57) / 69,81(12,14). La evaluación con análisis de varianza mostró el efecto significativo de las variables material, profundidad y momento del registro (p<0,05) y la prueba de Tukey mostró que los sitios de registro (profundidad) fueron estadísticamente significativos, dando cuatro grupos homogéneos. Bajo las condiciones de este estudio podemos concluir que la microdureza de las resinas de inserción en bloque se ve afectada por el nivel de profundidad y el tiempo pos curado.

INTRODUCCIÓN: La baja sensibilidad y especificidad de las ayudas diagnósticas y el bajo aislamiento en los cultivos dificulta el reconocimiento de la sepsis bacteriana de inicio temprano en neonatos. OBJETIVO: Determinar la validez diagnóstica de la de la proteína C reactiva (PCR) en la sepsis en la sepsis neonatal temprana. MÉTODO: Se evaluó el papel de la PCR en el diagnóstico de sepsis neonatal temprana. Las concentraciones se midieron a las 12 y 48 h de vida en pacientes con sospecha de sepsis. Al evaluar la sensibilidad y especificidad de la PCR, se utilizó el resultado de manera cuantitativa y mediante una curva ROC no paramétrica se estimó sensibilidad y especificidad, razones de verosimilitud y porcentaje de clasificación correcta para cada punto de corte posible. RESULTADOS: El estudio incluyó 198 pacientes. La sensibilidad, especificidad, valor predictor positivo (VPP), valor predictor negativo (VPN), índice de probabilidad positiva e índice de probabilidad negativa de la PCR fue de 72,2 - 82,4 - 45,2 - 93,7 - 4,1 y 0,3, respectivamente con área bajo la curva de 0,78. CONCLUSIONES: La PCR es particularmente útil para descartar una infección. Dos PCR seriadas negativas en ausencia de expresión clínica y de hemocultivos positivos tienen un alto VPN y un índice de probabilidad negativa a favor de excluir la infección con una alta certeza y/o de descontinuar la terapia antibiótica.


BACKGROUND: The low sensitivity and specificity of diagnostic aids and the low isolation in cultures make it difficult to recognize early-onset bacterial sepsis in neonates. AIM: Determine the diagnostic validity of C reactive protein (CRP) in early neonatal sepsis. METHOD: The role of CRP in the diagnosis of early-onset neonatal sepsis was evaluated. Levels were measured at 12 and 48 hours of life in patients with suspected sepsis. When evaluating the sensitivity and specificity of the CRP, the result was used quantitatively, using a non-parametric ROC curve to estimate sensitivity and specificity, likelihood ratios and percentage of correct classification for each possible cut-off point. RESULTS: The study included 198 patients. The sensitivity, specificity, positive predictive value, negative predictive value, positive probability index and negative probability index of CRP, were 72.2 - 82.4 - 45.2 - 93.7 - 4.1, and 0.3, respectively with area under the curve of 0.78. CONCLUSIONS: CRP is particularly useful to rule out infection. Two negative serial CRP in the absence of clinical symptoms and positive blood cultures have a high negative predictive value and a negative probability index in favor of excluding infection with high certainty and/or discontinuing antibiotic therapy.

ABSTRACT

ABSTRACT The aim of this research was to determine compressive and shear bond strength of blocks prepared with bulk-fill and nanofill composite resin combinations. Materials used were Filtek Bulk Fill (FBF) and Z350 (both 3M-ESPE) and Surefil SDR flow (SFF) - Dentsply. To determine shear bond strength, cylindrical specimens 10 mm thick were prepared with composite consisting of thicknesses of 6 mm of one material and 4 mm of the other, in the following combinations: G1: FBF- FBF; G2: Z350-Z350, G3: FBF-Z350, G4: Z350-SFF and G5: SFFSFF. Materials were cured using a 1100 mw/cm2 light for 20 seconds for each layer. Samples were stored for 24 hours at 37 °C in distilled water and shear bond strength was determined. To assess compressive strength, cylindrical samples 4 mm diameter and 6 mm thick consisting of 4 mm + 2 mm were used in the same combinations as described above, stored in distilled water at 37 °C for 24 hours, after which compressive strength was determined. Both tests were performed with a Universal testing machine at a cross head speed of 1 mm/min. Results were analyzed with ANOVA and Tukey's test. Means and standard deviations in MPa for each group were the following: Shear bond strength: G1: 435.87 (65.86), G2: 233.6 (108.15), G3: 279.2 (22.05), G4:449.1 (109.35) and G5: 196.6 (51.16). Compressive strength: G1:160.07(4.27), G2: 149.49 (14.06), G3: 156.10 (29.99), G4: 199-30(39.28), G5: 171.23 (28.71). Evaluation with ANOVA showed no significant differences among combinations for compressive strength (p>0.05) and significant differences for bond strength (p<0.05). Tukey's test showed three homogeneous groups. Under these experimental conditions, it can be concluded that the study combinations have adequate mechanical behavior, equivalent to materials used individually. However, shear bond strength was affected by the combinations analyzed.


RESUMEN El objetivo de este trabajo fue determinar la resistencia compresiva (RC) y la resistencia adhesiva al corte (RAC) en bloques preparados con combinaciones de composites bulk-fill y nanoparticulados. Los materiales usados fueron Filtek Bulk Fill (FBF) y Z350 (ambos de 3M-ESPE) y Surefil SDR flow (SFF) - Dentsply. Para medir la RAC, se prepararon probetas cilindricas de 10 mm de espesor consistentes en 6 mm de un material y 4 mm del otro con las siguientes combinaciones: G1: FBF- FBF; G2: Z350-Z350, G3: FBF-Z350, G4: Z350-SFF y G5: SFF-SFF. Se curaron a 1100 mw/cm2 durante 20 segundos cada capa. Se conservaron 24 horas a 37 °C en agua destilada antes de determinar la RAC. Para medir la RC se prepararon probetas de 4 mm de diametro y 6 mm de espesor (4 mm + 2 mm de cada material), con las mismas combinaciones. Se conservaron en agua destilada a 37 °C durante 24 horas y se midió la RC. Ambos ensayos se realizaron con una máquina universal para ensayos mecánicos a 1 mm/min de velocidad de desplazamiento de cabezal. Los resultados se evaluaron con ANOVA y prueba de Tukey. Las medias y desviaciones estándar (MPa) para cada grupo fueron: RAC: G1: 435.87 (65.86), G2: 233.6 (108.15), G3: 279.2 (22.05), G4:449.1 (109.35) y G5: 196.6 (51.16). RC: G1:160.07(4.27), G2: 149.49 (14.06), G3: 156.10 (29.99), G4: 199-30(39.28), G5: 171.23 (28.71). ANOVA no mostró diferencias estadísticamente significativas para RC (p>0.05) y diferencias significativas para RAC (p<0.05). La prueba de Tukey mostró tres grupos homogéneos. En las condiciones experimentales de este trabajo puede concluirse que las combinaciones evaluadas tienen un comportamiento mecánico adecuado equivalente al de los materiales individuales. Sin embargo, la adhesión entre materiales se vio afectada por las combinaciones realizadas.

ABSTRACT

@#The Hutchinson-Gilford progeria syndrome (HGPS) is a rare genetic disease that involves singlebase gene mutation in the LMNA gene which results in the production of a dysfunctional and mutant lamin A protein called progerin. Progerin is found in increased concentration in normal older individuals hence patients present with phenotypic signs of aging. [1] Based on current studies, there is no established predisposition and association between abdominal masses, specifically ovarian masses in female, adolescent, progeria patients. This is an adolescent female patient with progeria presenting with an ovarian mass. Further studies to establish the correlation between Hutchinson-Gilford progeria syndrome (HGPS) and abdominal masses specifically masses in the reproductive system have yet to be done. The exact mechanism by which progeria patients become predisposed to developing abdominal masses, specifically ovarian masses is still a grey area in research. Through this case report, routine abdominal ultrasound screening or routine abdominal CT scan can be done to screen for presence of masses in HGPS patients.

OBJECTIVES: This study aimed to evaluate the potential anti-inflammatory effects of vitamin D supplementation under uremic conditions, both in vivo and in vitro, and its effects on the parameters of mineral metabolism. METHODS: Thirty-two hemodialysis patients were randomly assigned to receive placebo (N=14) or cholecalciferol (N=18) for six months. Serum levels of calcium, phosphate, total alkaline phosphatase, intact parathyroid hormone (iPTH), and vitamin D were measured at baseline and after three and six months. The levels of fibroblast growth factor-23 (FGF-23), interleukin-1β (IL-1β), and high-sensitivity C-reactive protein (hs-CRP) were also measured at baseline and at six months. Human monocytes were used for in vitro experiments and treated with cholecalciferol (150 nM) and uremic serum. Cell viability, reactive oxygen species (ROS) production, and cathelicidin (CAMP) expression were evaluated using the 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide assay, dichloro-dihydro-fluorescein diacetate assay, and real time-quantitative polymerase chain reaction, respectively. RESULTS: Both patient groups were clinically and biochemically similar at baseline. After six months, the levels of vitamin D and iPTH were higher and lower, respectively, in the cholecalciferol group than in the placebo group (p<0.05). There was no significant difference between the parameters of mineral metabolism, such as IL-1β and hs-CRP levels, in both groups. Treatment with uremic serum lowered the monocyte viability (p<0.0001) and increased ROS production (p<0.01) and CAMP expression (p<0.05); these effects were counterbalanced by cholecalciferol treatment (p<0.05). CONCLUSIONS: Thus, cholecalciferol supplementation is an efficient strategy to ameliorate hypovitaminosis D in hemodialysis patients, but its beneficial effects on the control of secondary hyperparathyroidism are relatively unclear. Even though cholecalciferol exhibited anti-inflammatory effects in vitro, its short-term supplementation was not effective in improving the inflammatory profile of patients on hemodialysis, as indicated by the IL-1β and hs-CRP levels.

ABSTRACT

ABSTRACT Objective: To validate the defining characteristics of the nursing diagnoses, impaired memory and chronic confusion for older adults, by testing diagnostic concept definitions among expert nurses. Methods: We used a Diagnostic content validation using an online survey of expert clinical nurses. Results: 195 expert nurses performed the diagnostic validations. Findings provided validity of impaired memory with 11 major defining characteristics and chronic confusion, with 11 major and one minor defining characteristics. In both diagnoses, content validity index was 0.85. Factor analysis provided four and five supported factors for impaired memory and chronic confusion, respectively. Conclusion: The study provided evidence of validity of the two diagnoses and made them clearer. Using these updated nursing diagnoses, nurses have the potential to improve accuracy and quality of care for elderly patients, contributing to more accurate nursing gerontological care.


RESUMO Objetivo: Validar as características definidoras dos diagnósticos de enfermagem memória prejudicada e confusão crônica para idosos, testando as definições dos diagnósticos entre enfermeiros especialistas. Método: Validação de conteúdo diagnóstico utilizando um questionário online para enfermeiros clínicos especialistas. Resultados: 195 enfermeiras especialistas realizaram as validações diagnósticas. Os resultados demonstraram a validade de memória prejudicada com 11 características definidoras principais, e confusão crônica com 11 características definidoras principais e uma secundária. Em ambos os diagnósticos, o índice de validade de conteúdo foi de 0,85. A análise fatorial forneceu quatro para memória prejudicada e cinco fatores para confusão crônica. Conclusão: O estudo forneceu evidências de validade dos dois diagnósticos, tornando-os mais claros. Por meio do uso desses diagnósticos de enfermagem atualizados, os enfermeiros podem melhorar a precisão e a qualidade do atendimento a pacientes idosos, contribuindo para um cuidado gerontológico de enfermagem mais preciso.


RESUMEN Objetivo: Validar las características que definen los diagnósticos de enfermería memoria deteriorada y confusión crónica en adultos mayores, testeando las definiciones de los diagnósticos entre enfermeros especialistas. Método: Se trata de una validación del contenido diagnóstico realizado mediante un cuestionario en línea para enfermeros clínicos especialistas. Resultados: Las validaciones de diagnóstico las llevaron a cabo 195 enfermeras especialistas. Los resultados demostraron la validez del deterioro de la memoria con 11 características principales definidoras y la confusión crónica, con 11 principales definidoras y una secundaria. En ambos diagnósticos, la tasa de validez del contenido fue de 0,85. El análisis factorial proporcionó cuatro factores para el deterioro de la memoria y cinco para la confusión crónica. Conclusión: El estudio trajo evidencias de la validez de los dos diagnósticos y los dejó más claros. Al valerse de estos diagnósticos actualizados, los enfermeros pueden mejorar la precisión y la calidad de la atención de los pacientes adultos mayores y contribuir aún más con el cuidado gerontológico de enfermería.

The major glycoproteins of bovine gammaherpesvirus 4 (BoHV-4) are gB, gH, gM, gL, and gp180 with gB, gH, and gp180 being the most glycosylated. These glycoproteins participate in cell binding while some act as neutralization targets. Glycosylation of these envelope proteins may be involved in virion protection against neutralization by antibodies. In infected cattle, BoHV-4 induces an immune response characterized by low neutralizing antibody levels or an absence of such antibodies. Therefore, virus seroneutralization in vitro cannot always be easily demonstrated. The aim of this study was to evaluate the neutralizing capacity of 2 Argentine BoHV-4 strains and to associate those findings with the gene expression profiles of the major envelope glycoproteins. Expression of genes coding for the envelope glycoproteins occurred earlier in cells infected with isolate 10/154 than in cells infected with strain 07/435, demonstrating a distinct difference between the strains. Differences in serological response can be attributed to differences in the expression of antigenic proteins or to post-translational modifications that mask neutralizing epitopes. Strain 07/435 induced significantly high titers of neutralizing antibodies in several animal species in addition to bovines. The most relevant serological differences were observed in adult animals. This is the first comprehensive analysis of the expression kinetics of genes coding for BoHV-4 glycoproteins in 2 Argentine strains (genotypes 1 and 2). The results further elucidate the BoHV-4 life cycle and may also help determine the genetic variability of the strains circulating in Argentina.

ABSTRACT

El propósito de este trabajo fue determinar una vinculación entre la permeabilidad de las vías aéreas superiores, con la clase II dentaria en pacientes que hayan completado su crecimiento. Los parámetros utilizados para determinarlo fueron: el cefalograma de Mc Namara en lo que respecta a vías aéreas superiores y la posición del primer molar superior según la clasificación propuesta por Angle. Los datos para evaluarlo se obtienen mediante una teleradiografía lateral de cráneo o pruebas que pueden realizarse incluso en el consultorio. La muestra del presente trabajo estuvo conformada por 100 teleradiografías de pacientes con clase II dentaria. La obstrucción respiratoria que presentan determinados pacientes podría considerarse como un factor predisponente en el desarrollo de la formación de una maloclusión, aunque no necesariamente todos los insuficientes respiratorios presentan algún tipo de anomalía dentoesqueletal, este vínculo entre forma y función resulta muy importante a la hora del diagnóstico, pronóstico y la planificación del tratamiento


The purpose of this study was to determine a link between upper airways permeability and dental class II in patients who have finished their growth. The parameters used for establishing them were: Mc Namara's cephalometrics with regard to the upper airways, and the position of the first upper molar according to Angle's classification. The information to evaluate it was obtained either via lateral teleradiography of the skull or tests which can be done in the office. The sample was composed of 100 teleradiography of patients with dental class II. The breathing obstruction could be considered a predisposing factor in the increase of the formation of a bad occlusion, yet not every insufficient breather has some kind of skeletal tooth anormaly; thus, establishing the link between shape and function is a determining factor at the time of giving the correct diagnosis and prognosis, and of deciding on the best treatment

ABSTRACT The aim of this study was to evaluate the effect of flowable composite or glass ionomer liners on the shrinkage stress of a restorative composite resin. Fifteen previously sandblasted metal boxes were attached to a universal mechanical testing machine (INSTRON 1011, Instron Corporation). Five of these boxes were filled with Filtek Z350 XT (FXT) Universal Restorative A2 (3M ESPE) (Group 1 or Control). Two further groups of 5 boxes were prepared by interposing a layer of Vitrebond Light Cure Glass Ionomer 3M ESPE (VGI) (Group 2 or G.I.) or Filtek Z350 XT Flowable Restorative A2 3M ESPE (FFR) (Group 3 or Flowable) between the box and the composite resin, completing with the same volume of composite as in Group 1. Upon activating lightcuring, the filled boxes mounted on the testing machine were videoed for 60 seconds (40 s photoactivation and 20 s postcuring), timed with a digital chronometer. Force values were recorded in newtons and converted into stress according to contact surface. Stress values were recorded every 10 s. Results were analyzed using repeated measures ANOVA. Mean and standard deviation in kPa (stress) recorded for each group were: Control group: 126.2 (30.8); G.I.: 48.4 (18); Flowable: 27.9 (19.5). Statistical analysis showed significant differences between the control group and the rest (p<0.01), with no significant difference between groups with glass ionomer liners and flowable resin liners (G.I. and Flowable). Under the experimental conditions of this study, it can be concluded that polymerization shrinkage stress can be reduced by the presence of a liner between the preparation and the restorative material.


RESUMEN El objetivo de este trabajo fue evaluar el efecto de la colocación de una capa de Composite flow o Ionómero vítreo sobre la tensión de contracción de un composite para restauración. Se utilizaron 15 cajas metálicas previamente arenadas y conectadas a la máquina universal para ensayos mecánicos (INSTRON 1011, Instron Corporation). Cinco de estas cajas (G1) se rellenaron con Filtek Z350 XT (FXT) Universal Restorative A2 3M ESPE. Al iniciar la activación de la unidad de curado se comenzaba a registrar con una cámara de video y un cronómetro digital desde el comienzo de la activación de la lámpara hasta 60 s después, registrando los valores post curado durante 20 s. Los valores de fuerza generados por la polimerización fueron registrados en newton de cada 10 s para los 15 ensayos. Los valores fueron convertidos en tensión de contracción según la superficie de contacto. Se realizaron además dos grupos de cajas (5 en cada una) en los cuales se colocaron una capa inicial de Vitrebond Light Cure Glass Ionomer 3M ESPE (VGI) (G2 o IV) y Filtek Z350 XT Flowable Restorative A2 3M ESPE (FFR) (G3 o Flow) y se completó con el mismo volumen de composite de las del GI. Los resultados obtenidos fueron analizados por medio de ANOVA para mediciones repetidas. La media y la desviación estándar en kPa (tensión o estrés de contracción) registrado para cada grupo fueron: Grupo control: 126.2 (30.8); IV: 48.4(18); Flow: 27.9(19.5). El análisis estadístico mostró diferencias estadística mente significativas entre el grupo control y el resto (p=0.00), pero no hubo diferencias significativas entre la presencia de Ionómero vítreo o Composite Flow (IV y Flow). En las condiciones experimentales de este trabajo puede concluirse que la tensión de contracción generada durante la polimerización puede ser disminuida por la presencia de algún material interpuesto entre la preparación y el composite restaurador.

ABSTRACT

INTRODUCCIÓN La peritonitis meconial (PM) es una peritonitis localizada o generalizada, aséptica, química o de cuerpo extraño; producto del paso de meconio a la cavidad peritoneal y esta correlacionada con la perforación prenatal del tracto digestivo. Se presenta en 1 de cada 30.000 recién nacidos (RN). El diagnostico ecográfico prenatal mejora los resultados perinatales, el hallazgo más frecuente es la ascitis. La PM requiere un tratamiento multidisciplinario urgente, la mayoría es de resolución quirúrgica. OBJETIVO Dar a conocer una patología infrecuente, que requiere un alto grado de sospecha diagnostica para otorgar un manejo perinatal específico y oportuno. Caso clínico Primigesta de 29 años sin antecedentes mórbidos, cursando embarazo controlado de 36+5 semanas. En control ecográfico se evidencia ascitis fetal. Se hospitaliza en alto riesgo obstétrico, descartando patología metabólica e infecciosa. A las 37 semanas por cesárea de urgencia, se obtiene RN con distensión abdominal y hepatomegalia. En laparotomía exploradora se evidencia asas intestinales indemnes. En re intervención se encuentra hernia de íleon distal perforada, se confecciona ostomia, evoluciona favorablemente y es dado de alta. DISCUSIÓN Es fundamental considerar la PM dentro de los diagnósticos diferenciales de ascitis fetal. Un diagnóstico oportuno mejora los resultados perinatales y permite prevenir posibles complicaciones.


INTRODUCTION Meconial peritonitis (PM) is localized or generalized peritonitis, aseptic, chemical or strange body; a product of meconium steps to the peritoneal cavity and is correlated with prenatal perforation of the digestive tract. It occurs in 1 in 30,000 newborns. Prenatal ultrasound diagnosis improves perinatal outcomes, the most frequent finding is ascites. PM requires urgent multidisciplinary treatment, most of cases need surgical resolution. OBJECTIVE To present an infrequent pathology, which requires a high degree of diagnostic suspicion to grant a specific and timely perinatal management. Clinical case Pregnant 29 years old woman without morbid history, 36 weeks of controlled pregnancy. Ultrasound control shown fetal ascites. He is hospitalized at high obstetric risk, ruling out metabolic and infectious pathology. At 37 weeks by emergency caesarean section, is obtained a baby boy with abdominal distension and hepatomegaly. In exploratory laparotomy there are undamaged intestinal handles. In re intervention it is noted herniated perforated distal ileum, ostomy is made. Patient evolves favourably and is discharged. DISCUSSION It is essential consider PM within the differential diagnoses of fetal ascites. An opportune diagnosis improves the perinatal results and could avoids possible complications.

ABSTRACT

Las Clases III están caracterizadas por una pérdida de dimensión vertical, asociada a un acortamiento de las inserciones y desbalance musculares. Este estudio fue diseñado para determinar los cambios en la amplitud de la electromiografía (EMG) de la actividad muscular de los maseteros (M) y temporales (T) en reposo y en apretamiento en pacientes con Clase III, tratados neurofisiológicamente con electroestimulación nerviosa transcutánea (Transcutaneous Electrical Nerve Stimulation - TENS), utilizando un dispositivo intraoral (DIO) las 24 horas durante 30 días. Doce pacientes con Clase III molar (edad promedio: 27,7+-1,2 años) formaron parte del grupo tratado con TENS y DIO, y 10 pacientes con edades similares de Clase III, sin tratamiento, conformaron el grupo control. La EMG muscular de M y T fue estudiada antes y después del tratamiento. Los datos fueron analizados utilizando la prueba "t" de Student; adicionalmente, se calculó la media y el desvío estándar de la actividad de cada músculo. Los pacientes tratados con TENS y DIO presentaron una disminución marcada de la hiperactividad en reposo de los M y T (p=0,0001 - asociación altamente significativa - A/S) y un aumento de la misma en apretamiento (p=0,0001 A/S). El grupo control mantuvo similar actividad en reposo y en la EMG en apretamiento, mostrando asociaciones no significativas. Estos resultados confirman que utilizar el TENS y el DIO en pacientes mejora la actividad de los músculos M y T , disminuyendo la hiperactividad en reposo y mejorando la misma en apretamiento. De este modo cambiaría sustancialmente la posición inicial de la oclusión para tratar las maloclusiones de Clase III, lo cual facilita el diagnóstico, pronóstico y plan de tratamiento.


Classes III are characterized by a loss of vertical dimension associated with a shortening of the muscle attachments and imbalance. This study was designed to stablish the changes in the amplitude of the electromyogram (EMG) of temporal (T) and masseter (M) muscle activity at rest and during clenching in patients with Class III, neurophysiologically treated with Transcutaneous Electrical Nerve Stimulation (TENS), using an intraoral device (DIO) 24 hours for 30 days. Twelve patients with Class molar relation (mean age, 27.7+-1.2 years) were part of the group treated with TENS and DIO and 10 untreated patients with similar ages and Class III, formed the control group. The muscle EMG of M and T was studied before and after treatment. Data were analyzed using the Student´s test, additionally, mean and standard deviation of the activity of each muscle was calculated. Patients treated with TENS and DIO showed a marked decrease in hyperactivity of M and T at rest (p=0.0001 A/S) and an increase of it during clenching (p=0.0001 A/S). The control group kept similar activity at rest and in the EMG during clenching, showing no significant associations. These results confirm that using TENS and DIO in patients improves muscle activity of T and M as decreasing hyperactivity at rest and improving it during clenching. Thus, the initial position of the occlusion to treat Class III malocclusions should substantially change, thereby providing diagnosis, prognosis and treatment plan.

ABSTRACT

Gonococcal keratoconjunctivitis is a rapidly progressing and aggressive infection caused by Neisseria gonorrhoeae . We report a case of a patient who presented keratoconjunctivitis with an opacity in the left cornea that progressed into an ulcerative lesion despite initial treatment with antibiotic eye drops. Gram stains from the purulent discharge of the left eye showed gram-negative diplococci, and the culture from the ocular discharge was positive for Neisseria gonorrhoeae . Resolution was achieved with the administration of 2 g of intramuscular ceftriaxone in a single dose, and the patient had no sequelae.


La queratoconjuntivitis gonocócica es un infección agresiva y de rápida progresión causada por Neisseria gonorrhoeae . Reportamos el caso de un paciente quien presentó queratoconjuntivitis con opacidad corneal izquierda, la cual progresó a lesión ulcerativa a pesar del tratamiento inicial con antibiótico en gotas oftálmicas. La tinción de Gram y el cultivo a partir de la secreción purulenta del ojo izquierdo mostró diplococos gramnegativos y crecimiento de Neisseria gonorrhoeae , respectivamente. La curación del paciente se logró tras la administración de 2 g de ceftriaxona intramuscular en dosis única; el paciente no presentó secuelas.

Evaluar las características operativas del diagnóstico clínico (síntomas y signos) solo y con pruebas de diagnóstico en el consultorio (pH y prueba de aminas), teniendo como patrón de oro el puntaje de Nugent, para el diagnóstico de vaginosis bacteriana.Materiales y métodos: estudio de validez diagnóstica, ensamblado en un estudio de corte transversal en pacientes con síntomas de infección del tracto genital inferior, quienes consultaron a tres centros de Bogotá en 2010. Se calculó un tamaño de muestra de 1400 participantes. Se realizó un muestreo secuencial. Se tomaron muestras de la pared vaginal para medición de pH, prueba de aminas y tinción de Gram para puntaje de para puntaje de Nugent, considerado como patrón de oro de la entidad. Las bacteriólogas estaban enmascaradas con respecto al diagnóstico clínico y a las pruebas de consultorio. Se midieron la sensibilidad y especificidad, razón de probabilidades RP (+) y RP (-). Se presentan como proporciones con su respectivo intervalo de confianza del 95%.Resultados: la prevalencia de VB fue de 39.6 %, las características operativas de las pruebas con sus respectivos IC 95 %, con el puntaje de Nugent como patrón de oro, fueron: diagnóstico clínico, sensibilidad 75 % (71-78), especificidad 54 % (51-57), pH≥ 5 sensibilidad 99,3 % (98,2-99.8), especificidad 0,57 % (0,2-1,3); prueba de aminas, sensibilidad 99,3 % (98,2-99,8 %), especificidad 99,8 % (99,2-100 %); diagnóstico clínico, más prueba de aminas,más pH ≥ 5,0, sensibilidad 74 % (71-78), especificidad 100 % (99-100).Conclusiones: el diagnóstico clínico es una estrategia sensible para el diagnóstico de VB; la especificidad mejora cuando se combina con la prueba de aminas y el pH vaginal. La prueba de aminas presentó excelente sensibilidad y especificidad, y es útil para mejorar el diagnóstico de VB...


To assess the operational characteristics of the clinical diagnosis (signs and symptoms) alone and with diagnostic tests in the office (pH and amine test), using the Nugent score as the gold standard for diagnosing bacterial vaginosis.Materials and methods: Diagnostic validity study assembled in a cross sectional study in patients with symptoms of lower genitourinary tract infection seen in 3 centres in Bogota in 2010. The calculated sample size was 1400 participants, and a sequential sampling was used. Samples were taken from the vaginal wall for pH measurement, amine test and Gram staining in order to derive the Nugent score, considered the gold standard in this disease. The lab technicians were blinded to the clinical diagnosis and to the office tests. Sensitivity, specificity ad odds ratio OR (+) and OR (-) were measured, and they are presented as proportions with their respective 95% confidence intervals.Results: The prevalence of bacterial vaginosis was 39.6 %, and the operational characteristics of the tests with their respective 95 % CI were as follows: sensitivity and specificity of the clinical diagnosis were 75 % (71-78) and 54 % (51-57), respectively; sensitivity and specificity of pH ≥ 5 were 99.3 % (98.2-99.8), and 0.57 % (0.2-1.3); amine test sensitivity 99.3 % (98.2-99.8 %) and specificity 99.8 % (99.2-100 %); clinical diagnosis plus amine test and pH ≥ 5.0, sensitivity 74 % (71-78), specificity 100 % (99-100).Conclusions: Clinical diagnosis showed a good sensitivity for diagnosing strategy for diagnosing bacterial vaginosis; specificity is improved when clinical findings are combined with amine test and vaginal pH. ABSTRACT

El herpesvirus bovino 4 [Bovine herpesvirus 4 (BoHV-4)] ha sido aislado de bovinos con infecciones respiratorias, vulvovaginitis, mastitis, abortos, endometritis y de animales aparentemente sanos en diferentes partes del mundo. Si bien no se ha reconocido como agente causal de una entidad patológica en particular, se asocia principalmente con infecciones del tracto reproductivo de los bovinos. Este virus puede infectar un amplio rango de especies tanto in vivo como in vitro. Los primeros aislamientos dieron origen a dos grupos de cepas prototipo: el grupo americano tipo DN599 y el grupo europeo tipo Movar. En Argentina, el BoHV-4 fue aislado y caracterizado en el año 2007; este aislamiento se obtuvo de muestras de mucus cérvico-vaginal de vacas que abortaron. Hasta el momento se han registrado más de 40 aislamientos, provenientes principalmente de hembras bovinas que han abortado


Bovine herpesvirus 4 (BoHV-4) has been isolated from cattle with respiratory infections, vulvovaginitis, mastitis, abortions, endometritis and from apparently healthy animals throughout the world. Although it has not yet been established as causal agent of a specific disease entity, it is primarily associated with reproductive disorders of cattle. This virus can infect a wide range of species, either in vivo or in vitro. Two groups of prototype strains were originated from the first isolates: the DN599-type strains (American group) and the Movar-type strains (European group). In Argentina, BoHV-4 was isolated and characterized in 2007 from vaginal discharge samples taken from cows that had aborted. So far, more than 40 isolates, mainly associated with aborting bovine females have been registered in our country
